Handling of personal information by MIYAVI Fan Club "MYV CREW"

The handling of members' personal information in the fan club shall be done in accordance with
"Personal Information Protection Policy" and "Privacy Policy"separately specified by LDH JAPAN Inc.
In addition, the fan club will provide members’ personal information to The Dai-ichi Life Insurance Company, Limited and Sompo Japan Insurance Inc. for various types of guidance regarding the LDH PROTECT*.

In order for you to receive LDH PROTECT services, LDH will provide your personal information stated in ① below to the third parties stated in ②, and ② will use the personal information within the scope of purpose of use stated in ③.

• ③ LDH　PROTECT services
  Guidance of accident insurance for facility visitors, insurance payment, etc.
